Chris, firstly get out the soap and water and give them a good scrub, dirt and grease build up on the grip and reduce their effectiveness.

Once clean you can really see if they are worth changing.  If they are cracked, split, smooth or you cannot keep hold of them because the slip, then it is time to replace.

There is one other reason to replace(in my personal opinion); if you cannot stand the feel of the grip. I recently bought a 2nd hand wedge that had a Londridge grip on it.  Nothing actually wrong with the grip but all my others are Golf Pride and the Longridge did not feel right.  £10 later and a new Golf Pride grip was fitted the club feels fine.

Not overly scientific I know but I believe that much has to do with the feel and the resulting confidence you get.  We all know how much of this game is played in your head, why knock you confidence for the sake of a few grips.

Dean - I'd go along with cleaning them first, but have a go with a mild sugar soap solution first and rinse off thoroughly in clean water.  You'd be amazed what comes off.  If they're still horrible after that you could always have a go at re-gripping them yourself - it's really easy and cheap.
