Handicap Card Request??

I have tried to request a new card, yet there is no request link? all i get from the Home page is "Request new card" which when I click on it gives the following;
Request no. 105-Jun-200805-Jun-200805-Jun-2009
But there is nothing at the righthand side under "Request new card" to do the requesting???
